Pharmacist (Geriatric PACT)
The Clinical Pharmacist - Geriatric Patient Aligned Care Team (Geri PACT) practices in both the outpatient pharmacy and a clinical setting. They are responsible for providing pharmacy services to the outpatient Geri PACT practice setting within Geriatric and Extended Care (GEC) Service and other PACT teams and specialty care services. They are responsible for a major area of interest and practice in the outpatient PACT setting as an integral member of multidisciplinary teams. Duties include, but are not limited to: Clinical: Provides patient-centered care, and provision of drug information service to the outpatient Geri PACT, PACT clinics, and to other areas as requested.
