Job Summary:
We are seeking a CLINICAL NURSE II to be an integral member of the healthcare team to provide nursing services to patients and families in acordance with the scope of the RN as defined by the North Carolina Board of Nursing. This nurse will provide concierge care to patients in the Primary Care Medical Home Setting (Signature Care) and Health and Wellness Program (Executive Health).
Job Duties and Responsibilities:
Participate/ lead clinic daily huddles and actively engage in problem solving via Duke Quality System.
Perform patient intake assessment and documentation utilizing double identifiers so that patient is ready to be seen by provider on time (Reason for visit, Vital Signs, Medication Review, Falls Risk, Goals, Learning Assessment, PHQ2/9, SDOH, Patient Screening for Immunizations, appropriate forms, etc.)
Participate in own professional development by maintaining required competencies, assessing learning needs, and continuing education as required by the NCBON/ other Certification Agencies.
Maintain and adhere to current knowledge of polices/procedures as set forth by the PDC, other regulatory agencies, and those that are clinic specific. (Infection Control, Environment of Care, Joint Commission/ PCMH, etc.)
Promptly address In-basket tasks, Staff Messages, Patient Prescription Refill Requests, Patient calls, Prior Authorizations, faxing, etc. during work shift
Perform care tasks within nursing scope (includes, but not limited to, EKG's, ear lavage, medication administration utilizing Six Rights, Immunizations, nebulizer treatments, suture removal, urinary catheterization, wound dressings, PIV insertion)
Perform Phlebotomy, Point of Care Testing, and other lab services utilizing appropriate policy and procedure guidelines (Laboratory, Infection Control, etc.)
Perform equipment checks/controls; maintain accurate clinic logs with required documentation (ie: medication/lab/emergency equipment, refrigerators, etc.)
Monitor inventory, clean/stock exam rooms, check for expiration dates; report any issues to charge nurse, lead RN, or clinic management.
Perform Triage (walk-in and telephone): effectively assess acute patient needs, document utilizing available triage protocols, and communicate with care provider in a timely manner
Label patient documents for scanning into Maestro.
Participate in clinic or entity level Performance Improvement activities.
Maintain orderly work environment by keeping work station clean/tidy, hallways and patient care areas hazard free.
Educate the patient/ family/ support persons regarding medications, treatment, plan of care; document in the Electronic Health Record (EHR)
Delegate tasks/supervise activities of unlicensed assistive personnel; Assist other nursing personnel in the delivery of patient care; Assist with onboarding new clinical staff.
Coordinate patient movement throughout clinic to ensure smooth workflow, avoid bottlenecks, and completion of procedures in a timely fashion.
Provide continuous excellent customer service in a concierge care practice; explain delays, perform service recovery when needed or escalate to appropriate management when necessary.
Perform other related duties incidental to the work described herein.
Job Eligibility Requirements:
Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date.
Twelve months of appropriate clinical experience is required.
Must have current or compact RN licensure in the state of North Carolina. BLS required.
Job Schedule:
The schedule for this position is day shifts, Monday-Friday, with no weekends or holidays.
