Pharmacist - Home Care - Home InfusionSUMMARY: This Valley Children's Home Care position is accountable for providing safe and effective pharmaceutical care in the homecare setting. Serves as pharmacology consultant, a patient advocate, and a fiscal conscience to the healthcare team while providing clinical services to meet the needs of patients/care providers, physicians, nurses, and other healthcare team members. Accountable for processing safe and effective drug therapy to include review of physician's orders, administration and outcome of the drug therapy and provision of clinical pharmaceutical services. The emphasis of this position is advanced pharmacy practic
