The Clinical Pharmacist is responsible for managing multiple complex programs such as medication protocols, therapeutic drug monitoring, adverse drug reactions, medication reconciliation, medication histories and medication counseling for outpatients and inpatients in acute, subacute, and extended care units. Has knowledge and skills necessary to provide care appropriate to the age of the patient served. This includes knowledge of the changes associated with aging and the ability to incorporate these factors into the treatment needed by different age groups.
