The Clinical Pharmacist is responsible for managing multiple complex programs such as medication protocols, therapeutic drug monitoring, adverse drug reactions, medication reconciliation, medication histories and medication counseling for outpatients and inpatients in acute, subacute, and extended care units. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOFEL iBT). Initiates, continues, modifies, discontinues and otherwise orders drug therapy for both inpatients and outpatients only in accordance with an approved scope of practice, health care system protocols, and policies and procedures. Has knowledge and skills necessary to provide care appropriate to the age of the patient served. This includes knowledge of the changes associated with aging and the ability to incorporate these factors into the treatment needed by different age groups.
