This Job Opportunity Announcement will be used to fill a Clinical Pharmacist (Long Term Care), GS-0660-12 position in Pharmacy Service at the VA Montana Health Care System, Montana Veterans Home, Columbia Falls, MT. Basic Requirements: Citizenship: Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16 English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations: . In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates. GS-12 Clinical Pharmacist (Full Performance Level) Experience or Education: In addition to the basic requirements, candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. Knowledge, Skills, Abilities (KSAs): Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities. Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Physical Requirements: Physical movements involve completion of routine pharmacy functions which include but are not limited to: significant computer work/typing involving documenting patient encounters in the Computerized Patient Record System (CPRS), prescription order entry and processing, contact of patients (both face to face and via phone). Certain tasks are repetitive in nature and require multiple steps performed in succession for success. The incumbent should be able to lift and carry up to 50 pounds without significant difficulty. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging. ["VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws\nThe primary responsibility of the Columbia Falls Long Term Care Clinical Pharmacist is the provision of evidence-based pharmaceutical care services for all residents of the Montana Veterans Home (MVH) in Columbia Falls through both direct and indirect interaction with patients and providers, functioning with minimal supervision. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN and local levels. S/he must be capable of using professional judgment and clinical expertise to solve complex problems and to provide clinical information to both providers and Veterans. This pharmacist functions as part of the Montana VA Health Care System (MTVAHCS) Pharmacy Service to accomplish department missions and goals. The primary functions will be the interpretation and verification of physicians' orders, patient medication profile review (allergy, drug interaction screens, etc.), dispensing outpatient and long-term care medications, completing monthly long term care chart reviews, providing drug and policy information to patients and medical center staff, and participation in multidisciplinary meetings and educational programs within the service and medical center. Duties include, but are not limited to the following: Performs 30-day medication reviews for required patients in the MVH and for other patients as needed. Participates in Physician Rounds to consult on the planning of drug therapy; actively involved in reviewing patient medication regimens for drug selection, dosing, contraindications, side effects, potential drug interactions, cost effectiveness, and therapeutic outcomes as appropriate. Findings and recommendations for alternatives to current treatment plan will be communicated to providers as appropriate. Monitors for and reports medication errors, adverse drug reactions, allergies and patient compliance/adherence issues, documenting findings in accordance with facility policies and procedures. Reviews electronically submitted non-formulary drug requests utilizing National VA PBM Criteria for Use, completing review and documenting adjudication in CPRS in a timely manner as determined by VISN and National PBM leadership. Participates in discharge planning rounds and LTC evaluations; serves on the Extended Care QI Committee. Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing. Ensures all medication orders are entered into the electronic patient medical record. Fills all orders in a timely fashion; review of the patient's allergy history, drug-drug, drug-nutrient, drug-laboratory and drug-disease state interactions and assessment for clinical relevance should be performed and dealt with appropriately. Maintains complete and accurate records in all areas (outpatient, Unit Dose, IV) Possesses and demonstrates the ability to utilize knowledge of IV drug compatibilities in various solutions. Further possesses knowledge and is as able to communicate such knowledge in admixture solutions, irrigating fluids and IV sets. Possesses knowledge of the safe handling of hazardous substances and disposal of such. Participates in and/or supports Pharmacy Service-initiated cost/benefit programs in support of VA efforts at maintaining stewardship of resources. Performs as work leader for technicians, aids, volunteers, and students in their assigned area, ensuring that quality control measures as outlined in pharmacy policies and procedures are met. Effectively instructs patients and family members in the appropriate use of medications and medical devices and is able to adjust communication and tracking methods based on age, literacy level and learning barriers (e.g. low vision, hard of hearing, dementia, etc.).
