The Southern Arizona VA Health Care System Community Living Center Service is seeking (1) Registered Nurse for the Geri/Psych unit (GPU). * Evaluate the Veteran's condition and current status and develop individualized plan of care to address the particular needs of the Resident. *Use non pharmacological interventions to manage Dementia related challenging behaviors. * Coordinate Resident treatments with Physical, Occupational and Speech therapy. * Report and chart significant changes in resident condition, evidence of pain or motivational level. * Collaborate with the Interdisciplinary Treatment Team to deliver timely, efficient, effective care. * Perform ongoing monitoring of Resident conditions and document responses to care in a clear, concise and timely manner in CPRS * Interact with the family and/or significant others to facilitate Resident care. * Notify appropriate therapy discipline of problems, referrals, and reassessment needs. - Maintain consistent resident care schedule and be efficient in use of time. - Coordinate Restorative Treatments. * Deliver Nursing Care as a Charge Nurse. Lead a team consisting of an LPN, and 2 Nursing Assistants. * Precept and mentor new staff and student nurses. * Document treatment activity and summarize progress for each resident in CPRS weekly and as medical condition warrants. * Performs skills such as Vital signs checks, intravenous therapy, enteral tube feedings, wound care, ROM exercises, indwelling catheter care, respiratory care, CPR, ostomy care, tracheotomy care, medication administration, admissions and discharges. * Provide Resident education, help apply adaptive equipment and document all care that has been provided. This position is a direct patient care position requiring Basic Life Support to be attained and maintained throughout employment. Work Schedule: Full-time, 12 Hour Day Shift, Fluctuate, May include Weekends, Holidays. Financial Disclosure Report: Not required.
