You Will Be Responsible For:
Demonstrates complex assessment skills and the ability to coordinate the program to achieve optimal patient outcomes and operational efficiency by guiding the patients through and around barriers in the external and internal system.
Assumes responsibility as the clinical expert in the program of focus. Acts as a liaison between the patients, hospital, physicians, programs and services. Communicates effectively with physicians, management, staff, patients and families to ensure coordinated and cohesive care is maintained.
Effectively serves as a clinical resource for patients, physicians and other clinical services.
Communicates effectively with physicians, management, staff, patients and families to ensure coordinated and cohesive quality patient care in a timely manner. Consistently collaborates with members of the healthcare team to coordinate treatment plans, specific patient concerns and discharge planning. Accurately communicates the patient treatment goals and activities with the team members providing the care, participates in team conferences and provides information relating to therapeutic concerns in a clear concise manner.
Regularly visits and communicates with physicians to identify patient and physician needs in relation to the quality and ease of use of the program.
Conducts community assessments to identify and resolve systems barriers that interfere with timely access to needed care and services.
Identifies patient education needs to enhance patient care, customer satisfaction and quality outcomes in conjunction with the patient's physicians and other members of the healthcare team.
Develops/modifies and implements patient education programs/tools according to the principles of age specific learning. Individualizes patient's needs to enhance learning and promote continuous care throughout the treatment process.
Formulates individualized patient education by assessing the medical and social needs of the patient. Provides support and follow-up for all patients in the program.
Provides direct patient education to complex patients/families in collaboration with nursing staff, physicians and community resources. Establishes and maintains a therapeutic relationship with patients/families by explaining rationale for care and patient progression while demonstrating concern and awareness of their needs and communicates through documentation in the medical record/progress notes.
Identifies staff education needs to enhance associate satisfaction and quality patient outcomes for the processes within the program.
Evaluates the effectiveness and need for staff education at least quarterly to help staff understand our mission and purpose within the program.
Works with Social Services, healthcare providers, financial clearance, physicians and the patient's family to ensure the patient has access to available resources inside and outside of the hospital setting from diagnostic evaluation through treatment and rehabilitation. Maintains contact with the patient and outside care facilities to maximize the patient's care, strives to eliminate barriers to ensure the patient's care is not interrupted.
Assists in marketing initiatives for the program. Works collaboratively with Marketing department to maintain current and accurate information.
Develops/selects tools to monitor program performance improvement activities and clinical outcomes providing reports to the director in a timely manner. Assists with data abstraction and updating of reports to ensure compliance with standards of care for applicable patient population. Communicates and follows up with physicians and staff as necessary to assist with meeting compliance.
Attends, participates and serves as the chairperson as directed for committees or meetings; overseeing coordination of meetings and accurately documenting minutes as needed.
Reports to program director/manager problems or concerns involving direct/indirect patient care in a timely and professional manner, participates in performance improvement activities, coordinates efforts to address identified deficiencies.
Assumes responsibility for professional growth and demeanor, attends all mandatory nursing/hospital in-services, maintains and updates knowledge/skills in clinical area of specialization including specialized therapies and use of new/complex procedures/equipment and pharmacology applicable to clinical area of specialization. Maintains knowledge of current trends in medical therapy pertaining to area of clinical specialization through review of literature and attendance of professional programs, actively participates in service line development which promote optimal clinical outcomes and cost effectiveness in a collaborative forum with physicians and other members of the healthcare team. Provides leadership through clinical mentorship and role modeling.
What You Will Need:
Competent with Microsoft Office
Possess strong organizational skills with the ability to prioritize, manage and complete multiple tasks and projects within specified timeframes.
Age specific care for program of focus/clinical area of specialization.
Teaching and counseling patients, families and staff as needed.
Strong communication skills.
Minimum of 3-5 years of experience in clinical are of specialization required.
Clear/active FL license in clinical field required
AHA BLS required
Certification applicable to clinical area of specialization preferred.
Coordinates functions, including staff and community education, patient experience from diagnostic evaluation through treatment and rehabilitation. Monitors patient’s outcomes for quality of care. Prepares statistical reports and maintains required documentation for the patients and program. Identifies and resolves all potential barriers that the patient may face while in the hospital and during pre and post hospital care. Interprets program to physicians and other involved care providers. Develops relationships with referring physicians to understand care networks. Works with specialists to deliver efficient and effective patient care.
