The VA Northeast Ohio Healthcare System is recruiting for Nurse Practitioner. The Nurse Practitioner will function in Home Based Primary Care in Community Outpatient Service and be stationed primarily out of Akron, Parma or Wade Park (Cleveland). Note: This vacancy can be filled by a Nurse Practitioner or Physician Assistant. This position is ideal for a creative, dynamic, flexible, independent, clinically excellent Nurse Practitioner who communicates well with a diverse range of people, and is interested in a unique career combining the clinical care of medical patients with opportunities to contribute in other clinical, educational and quality improvement arenas. Duties include, but are not limited to: Serves Home Based Primary Care Northern and Southern Patient Aligned Care Teams as a float to provide additional coverage for complex and homebound/bedbound Veterans located throughout the Cleveland geographic area. Acts as the primary clinician for patient in the Home Based Primary Care program; entering orders and other documentation for Veterans; entering orders for specialized diagnostic studies; evaluating results of diagnostic studies and consultations; and initiating appropriate follow up. Obtains relevant health and medical history, completes documentation in the Veteran's record. Performs comprehensive physical examination, based on age and history. Performs psychiatric evaluations with assessment of potential for suicidal or homicidal ideation as needed. Acts as the primary clinician for on-going treatment of Veterans. Conducts preventative screening procedures based on age and history (including all mandated screening); educating Veterans regarding preventative health care. Identifies medical and health risks and needs assessment; screening Veterans to determine their need for further care. Initiates medical/surgical or appropriate referrals and consultations in accordance with medical practice and/or specified protocols. Formulates appropriate differential diagnosis based on history, physical examination and clinical findings. Identifies needs of the individual, family or community, based on evaluation of the data collected. Identifies, implements and evaluates appropriate plan of care, including pharmacologic and nonpharmacologic interventions. Conducts medical history to facilitate prescription of psychotropic medications. Provides relevant Veteran and family education. Refers to other health professionals and community agencies. Enters orders for Veterans including but not limited to: diagnostic studies, consultations, medications and supplies. Performs Phlebotomy as indicated/Administers immunizations/medications as indicated. Obtains informed consent for procedures, transfers, or opioid contracts. Creates and/or develops best practice standards. Other duties may be assigned at the discretion of Home Based Primary Care leadership.
